With hospitals and health systems increasingly facing value-based payments with incentives to keep patients healthy, many health care organizations are developing their own community health worker (CHW) workforce in the hopes that they will be able to improve outcomes and reduce the need for expensive emergency department and inpatient hospital care. This article describes contract design considerations for organizations developing a CHW workforce.
