With the prevalence of current asthma at over 10% in both adults and children, asthma remains a burden in Michigan despite advances in treatment. This paper provides comprehensive information about the MATCH model of in-home asthma case management, including outcomes from a recent evaluation of the model’s efficacy at three sites in Michigan, and details about how it has been replicated and sustained since 1996.
